Athletic Clearance

All athletic paperwork will be completed online for the 2023-2024 school year.

Sign up and complete all of the steps. Please turn in the following paperwork to Dion Coley, Athletic Director:

1) The certification form signed by parent and athlete, after all paperwork is complete
2) A copy of their insurance
3) Physical form signed and cleared by their doctor
